Hip Arthroscopy, labral debridement, acetabular rim recontouring for acetabular retroversion

The patient is a 19 year old active female with two years of pain and popping in the hip. On examination of the hip, she has pain with upward flexion (bending) of the hip greater than 90 degrees.

1.The standard anteroposterior radiograph of the hip shows the classic finding of "acetabular retroversion" known as the "cross-over-sign" where the anterior (front) wall of the hip socket overlaps the posterior (back) wall of the hip socket. This is clinically associated with impaction of the neck of the femur on the extended anterior wall of the hip socket leading to increased damage to the labrum. The anterior wall is shown in red and the posterior wall edge is shown in blue.

2.The CT scan images demonstrate the center line of the pelvis (yellow) and lines demonstrating the opening angle of the hip sockets (red). On both sides, the hip sockets open toward the back leading to "retroversion" or "turning back" of the hip socket.

3.The patient was treated with labral debridement (left image) and acetabular recontouring (right image). To see a video clip of the surgery, please click on either image.
